The referral process is simple

Once you as the care provider have identified a patient that would benefit from a LIFEBACK program, all you have to do is fill out the LIFEBACK Referral Form and fax it to 612/378-7526. We take it from there. If you want to see the steps we take with each referred patient, go to Getting Started.

The cost of the LIFEBACK program, including the follow-up evaluations, is usually covered by the patient’s health insurance plan because more than 400 insurance companies reimburse for LIFEBACK programs (we check this out for the patient). Insurance benefit levels can vary and we can help the patient determine and understand that benefit.
