It starts with a care provider referral
The information below describes the process by which a patient gains access to a LIFEBACK Spine Program. It starts with a referral form from your primary care provider.
- Obtain a prescription from your physician for one of the LIFEBACK Lumbar or Cervical Programs. The prescription is then sent to our offices for insurance coverage processing via fax (612) 378-7526. The physician's office can fax the completed form for you.
- Acceptance into the rehab program services starts with an initial evaluation. It determines your functional limits, your appropriateness for rehabilitation services and helps us to establish your individual program. The evaluation takes about 1.5 hours.
- If you're found appropriate, you'll be scheduled for the clinic phase of the LIFEBACK program at a Spine Rehab Clinic. You'll find we don't treat you, but train you and allow you time to master your LIFEBACK program. If not found appropriate, we return you to your primary care provider with our assessment.
- Upon completion of the clinic phase of your program, you will have created your managed home plan, a plan you'll devote some time to each day to continue your progress. At 30 days, you'll return to the clinic for a follow-up evaluation, about 1.0 hour. This assessment allows us to determine your progress and adjust your home program accordingly. Most patients will complete two follow-up evaluations and when they have met their goals are discharged. You will have a "healthy backs for life"® program that you can continue in your daily routine at home.
- Following your discharge, we will contact you at 3, 6, 12 and 24 months to assess your continued progress. There is no additional charge, and the opportunity allows you to ask any questions or receive the benefit of any new information our chronic low back clinicians might have. The cost of the LIFEBACK program, including the follow-up evaluations, is usually covered by your health insurance plan. We accept insurance benefit assignments from more than 400 payers.
